At the EADV Congress on September 30, 2026, Q32 Bio showed that all 12 SIGNAL-AA Part B patients with hair growth at Week 36 kept their response through 16 weeks without bempikibart, and its slides describe a Phase 3 pivotal trial planned for the first half of 2027. The evidence is still open-label Phase 2a data from 33 patients, and Nektar has set a competing 850-patient Phase 3 in the same window.

On September 30, 2026 Q32 Bio restated that it plans to advance bempikibart into a registration-directed program for severe or very severe alopecia areata in the first half of 2027, subject to regulatory discussions planned for the second half of 2026. The EADV slides describe it as a Phase 3 pivotal trial. No exact date, size or endpoint has been disclosed. September 30 release, SEC exhibit 99.1 · EADV slides
Of the 18 mITT patients who entered the 16-week off-drug period, 50% were at SALT≤20 at the last on-treatment visit and 44% at Week 52, with missing values carried forward. Among the 12 Week 36 responders, mean absolute SALT moved from 22.5 to 25.4, while all 12 met the study definition of maintained response (less than 20 points of worsening). In the mITT population, six patients who had not responded to their most recent oral JAK inhibitor showed a 4.8% mean SALT reduction, against 48.3% in four prior JAK responders. EADV slides · SEC exhibit 99.1
All 12 Week 36 responders kept their response 16 weeks off drug, biomarkers and biopsies fit the mechanism, and cash is guided through Phase 3 topline results.
Open-label design and 33 patients; off-drug SALT≤20 moved from 50% to 44%; little activity after JAK failure; Nektar’s Phase 3 starts in the same window.
June 30, 2026 cash and equivalents were $106.270 million; H1 operating cash use was $9.418 million. The July 16 offering added $187.6 million net. Their sum is a historical financing bridge, not October cash. Management guides resources through planned Phase 3 topline results, without a readout date. Clinical progress still depends on regulatory discussions and controlled replication of the open-label signal. Q2 Form 10-Q
Q32’s development case rests on early hair-regrowth activity, a selected off-drug responder analysis and a larger post-financing resource base. None establishes comparative efficacy or a final pivotal design. All 12 selected responders met a maintenance definition allowing less than 20 SALT points of worsening, while the broader off-drug cohort’s SALT≤20 proportion fell from 50% to 44%. The next decisions are the controlled program, maintenance schedule and regulatory requirements. Funding supports that work but arrives with substantial share-equivalent expansion; Nektar’s competing program also limits claims of unique positioning.
Leerink Partners initiated coverage of Q32 Bio at Outperform with a $40 price target, Dow Jones Newswires reported on October 12, 2026. A third-party analyst opinion, not a Merlintrader view and not a recommendation.
Nektar Therapeutics presented REZOLVE-AA Phase 2b data at EADV for rezpegaldesleukin, an injectable regulatory T-cell stimulator. ZENITH AA (850 patients, primary endpoint SALT≤20 at Week 52) is set to start in early 2027.
Q32 Bio presented additional SIGNAL-AA Part B results at EADV. Eighteen patients entered the 16-week off-drug follow-up; the 12 who had shown hair growth at Week 36 all maintained response under the study definition, and 42% deepened it.
The EADV slides (data cut-off September 25) state plans to initiate a Phase 3 pivotal trial in the first half of 2027 and predict a monthly-to-quarterly maintenance dose now being assessed in the open-label extension.
Q32 Bio appointed José Trevejo, M.D., Ph.D., as Chief Medical Officer and Ellie Hershberger, Pharm.D., as Chief Development Officer. The appointments are operational; they do not confirm that the registration program has started.

Open-label extension data, guided for H2 2027, strengthen the durability narrative shown at Week 52; FDA feedback supports a clean registrational path; the pivotal design preserves the favorable dose and safety profile; controlled efficacy approaches or exceeds the clinically meaningful Phase 2 signal. The enlarged balance sheet funds execution without another near-term equity raise, and bempikibart earns a credible position as a non-JAK first-line biologic.
The Phase 2 data justify late-stage development but remain exploratory. Q32 starts a controlled program in 1H27, burn rises materially and the market waits for placebo-adjusted evidence. The financing substantially reduces immediate funding pressure under the disclosed plan, while the larger denominator and long timeline limit enthusiasm until trial design and enrollment become clearer.
FDA feedback requires a larger, longer or more complex program than expected; durability is less consistent in the full dataset; controlled efficacy falls below the open-label signal or safety differentiation narrows. The company spends aggressively across populations or indications, and the substantial post-financing share base magnifies disappointment.
October 2 assessment: the September 30 off-drug data moved part of the bull case from hypothesis to observation, because every Week 36 responder kept the response for 16 weeks without dosing. The decisive variables, FDA requirements and controlled efficacy, are unchanged. Nektar’s planned 850-patient Phase 3 in early 2027 adds competitive pressure to the base and bear cases.
The reading on this page is that SIGNAL-AA Part B shows real but exploratory activity, that the July financing can carry bempikibart into a registrational program, and that the decisive evidence is still ahead. The observations below would contradict it.
None of these is a prediction. They are the observations that would make the rest of this page wrong, listed so that a reader can check them rather than take the reading on trust.
On July 13, 2026, Q32 Bio reported 36-week topline results from Part B of SIGNAL-AA, an open-label Phase 2a study of bempikibart in 33 patients with severe or very severe alopecia areata. The prespecified modified intent-to-treat (mITT) analysis showed a 35.3% mean reduction in SALT score; 10 of 25 mITT patients (40.0%) reached SALT-20, equal to 30.3% of all 33 enrolled patients.[1]
September 30 update. In a late-breaking oral presentation and a poster at the EADV Congress, Q32 Bio added the first off-drug data. Eighteen patients entered the 16-week off-drug follow-up; the 12 who had shown hair growth at Week 36 all maintained response under the study definition, and 42% deepened it. Across the 18, 44% were at SALT-20, 61% at SALT30 and 50% at SALT50 at Week 52. The release also reported Week 36 SALT30 and SALT50 rates of 44.0% in mITT and 33.3% in ITT. September 30 release, SEC exhibit 99.1
Two less flattering details belong in the same summary. Six mITT patients who had not responded to their last oral JAK inhibitor showed a 4.8% mean SALT reduction, against 48.3% in four prior JAK responders and 44.1% in JAK-naive patients. And within the off-drug cohort, SALT-20 moved from 50% at Week 36 to 44% at Week 52. The slides state a plan to initiate a Phase 3 pivotal trial in the first half of 2027 and predict a monthly-to-quarterly maintenance dose. EADV slides
On October 1, 2026, Nektar Therapeutics said its 850-patient ZENITH AA Phase 3 of rezpegaldesleukin, another non-JAK injectable, will start in early 2027, the same window as Q32 Bio’s program. Nektar release
The balance sheet is the other half of the story. Q32 Bio reported $106.3 million of cash at June 30, 2026, and the July 16 offering added $187.6 million of net proceeds ($200 million gross); management guides that these resources fund operations through topline Phase 3 results. The latest SEC cover shows 29,769,466 common shares at August 3, 2026. At the October 1 close of $8.25, basic equity value was approximately $245.6 million. The share price was below July’s $18.25 offering price.
The next phase is operational: regulatory discussions in H2 2026, the choice of pivotal dose and maintenance schedule, and a controlled program in H1 2027. The capital makes that plan credible; it does not guarantee that the signal reproduces against placebo.
Since the previous review the record changed in three ways. Q32 Bio filed a Form 8-K on September 30 under Items 7.01 and 8.01, so the EADV results are part of its SEC record. It posted the late-breaking slides (data cut-off September 25, 2026) and the poster (cut-off September 24, 2026), which contain the off-drug table, the prior-JAK split and the biomarker results analyzed below. And Nektar Therapeutics disclosed on October 1 the design and timing of a competing Phase 3. There has been no new Form 4 since August 27 and no new financing, and the ClinicalTrials.gov record for NCT06018428 still shows its May 8, 2026 update. The first balance sheet including the July proceeds will be the third-quarter report, for which no date has been announced.

Part B enrolled 33 patients with severe or very severe alopecia areata, defined by baseline SALT scores of 50 to 100 and a current disease episode lasting no more than four years. Prior oral JAK inhibitor use was permitted; 36.4% of enrolled patients had previously received an oral JAK inhibitor. Patients received 200 mg of subcutaneous bempikibart weekly for four loading doses and then 200 mg every other week for 32 additional weeks, for 36 weeks of active treatment. Off-drug follow-up continues through Week 52 before optional open-label-extension enrollment.[1]
Company-reported prespecified analyses. Percentages cannot be compared directly with other trials because designs, populations and time points differ.
Populations: mITT, n=25; all enrolled (ITT), n=33.
| Measure | Result | Interpretation |
|---|---|---|
| Mean SALT reduction | 35.3% in mITT | Primary efficacy analysis met the company’s target profile; a prespecified MMRM sensitivity analysis produced a 39.1% least-squares mean reduction. |
| Severe subgroup | 37.8% mean SALT reduction | Activity was stronger numerically in the severe subgroup than in the very severe subgroup. |
| Very severe subgroup | 27.4% mean SALT reduction | Signal remained present, but the subgroup contained only six mITT patients. |
| SALT-20 | 10/25 mITT; 10/33 ITT | Ten total patients achieved 80% or greater scalp hair coverage at Week 36. |
| SALT30 (30% relative improvement) | 11/25 mITT (44.0%); 11/33 ITT (33.3%) | Reported September 30; a lower bar than SALT-20. |
| SALT50 (50% relative improvement) | 11/25 mITT (44.0%); 11/33 ITT (33.3%) | Same counts as SALT30 at Week 36. |
| Prior JAK exposure | 36.4% of enrolled patients; 10 of 25 mITT | Mean SALT reduction 48.3% in four prior JAK responders, 4.8% in six prior JAK non-responders, 44.1% in JAK-naive patients. Exploratory, very small subgroups. |
| Duration of current episode | Under 2 years: 5 SALT≤20, -35.8%; over 2 years: 5 SALT≤20, -34.9% | Poster subgroup (12 and 13 mITT patients). |
The 25 mITT patients had a mean age of 41.9 years and a mean baseline SALT of 76.9; 76.0% were women, and 19 had severe and 6 very severe disease. Responders were spread across six sites in two countries. EADV poster
The response curve in the company presentation continued to deepen through Week 36. That supports the 36-week duration and raises the question of what maintenance schedule preserves the benefit.
The headline efficacy number is based on a 25-patient modified intent-to-treat population, not all 33 enrolled patients. The company presentation states that the mITT population excluded five early-termination patients who discontinued before Week 18, including two who had no post-baseline SALT assessment, and three patients with unstable disease under prespecified statistical-analysis-plan criteria. It also states that none of the five early terminations was related to bempikibart safety.[2]
This distinction is not a technical footnote. In the mITT group, 10 responders produce a 40.0% SALT-20 rate. Across all 33 enrolled patients, the same ten responders produce a 30.3% rate. Both numbers are valid for their stated analysis populations, but they answer different questions.
ITT denominator; the separate mITT rate is 10/25 = 40%.

September 30, 2026 update. The initial off-drug results are analyzed in the next section. In short, the 12 patients with hair growth at Week 36 all maintained response through 16 weeks without dosing, under a definition that allows less than 20 points of SALT worsening. This is not a 100% response rate across the 33-patient trial: selection, small numbers and the absence of a randomized control limit inference, and the dataset does not establish a long-term safety profile. September 30 release, SEC exhibit 99.1
In Part A’s open-label extension, eight patients entered after 26 to 55 weeks off treatment; management concluded that the totality of the data supports a maintenance regimen rather than very long drug-free intervals.
No new safety signals were reported in Part B, and the slides state that no patient discontinued because of a safety event. There were no treatment-related serious or Grade 3 or higher adverse events. Injection-site reactions were the most common treatment-emergent event, affecting 36.3% of patients, but represented approximately 4% of all administered doses. The company described all reported injection-site reactions as mild and self-resolving without intervention, with most resolving within one day; of the 12 patients with a reaction, eight had a single event.[1]
The poster lists the other common treatment-emergent events by severity. Upper respiratory tract infection was reported in 12 patients (seven mild, five moderate), gastroenteritis in five (one mild, four moderate), urinary tract infection in three (all moderate) and decreased lymphocyte count in three (all mild). Blocking IL-7Rα is expected to lower circulating T cells; the poster reports a mean fall in absolute lymphocyte count of about 40% that flattened at Weeks 27 to 28, with no correlation of infections with the decrease and no confirmed absolute count below 500 cells per microliter. The company documents do not agree on the window: the September 30 release says Weeks 24 to 28 and the slides Weeks 25 to 28. EADV poster · SEC exhibit 99.1 · EADV slides
The loading regimen achieved steady-state drug concentrations approximately ten weeks earlier than Part A: steady state by Week 3 rather than 12 to 14 weeks without loading, with a mean trough of 28.8 µg/mL at Day 22. Near-complete IL-7Rα occupancy on circulating CD3+ T cells was seen in more than 90% of patients by Day 15. The presentation reported roughly threefold higher trough concentrations during the first month versus Part A and negligible anti-drug antibodies without a meaningful effect on pharmacokinetics.
The readout does not establish placebo-adjusted efficacy, comparative efficacy versus an approved JAK inhibitor, long-term safety in a large population, a commercial maintenance schedule, pediatric efficacy, efficacy in moderate disease or a durable treatment-free remission rate.
The late-breaking slides (data cut-off September 25, 2026) show how the headline percentages were built. Of the 25 mITT patients, 18 entered the 16-week off-drug period (20 of the 33 enrolled patients on an ITT basis). Twelve of those 18 had improved by at least 10 SALT points by Week 36 and are the “responders” used for the maintenance analysis. The off-drug results therefore describe a selected group.[16]
| Measure | All mITT, Week 36 (n=25) | Off-drug cohort, Week 36 (n=18) | Off-drug cohort, Week 52 (n=18) |
|---|---|---|---|
| Mean percentage reduction in SALT from baseline | 35.3% | 45% | 47% |
| SALT≤20 | 40% | 50% | 44% |
| SALT30 | 44% | 56% | 61% |
| SALT50 | 44% | 56% | 50% |
Two readings are supported by the table. Sixteen weeks after the last injection, the cohort’s mean improvement was slightly larger and more patients had at least a 30% improvement. But the cohort started above the mITT average (50% at SALT≤20 versus 40%), and SALT≤20 slipped from 50% to 44% and SALT50 from 56% to 50%. Neither reading is a placebo-controlled estimate.
| Measure | Week 36 | 8 weeks off drug | 16 weeks off drug |
|---|---|---|---|
| Mean absolute SALT score | 22.5 | 22.9 | 25.4 |
| Maintained response | — | 12 (100%) | 12 (100%) |
| Deepened response | — | 6 (50%) | 5 (42%) |
“Maintained” is defined as less than 20 points of SALT worsening from the last on-treatment value, the same criterion the slides attribute to BRAVE-AA1, the baricitinib program. That is a generous threshold on a 100-point scale, which is why the mean absolute SALT of the responders could drift from 22.5 to 25.4 while all 12 still count as maintained. One patient reached SALT 0 at Week 44, two deepened to SALT≤10, and one latent responder reached SALT≤20 at Week 52 without further dosing. The company reads this as support for monthly-to-quarterly maintenance, a hypothesis now being tested in the extension.[16]
Ten of the 25 mITT patients had taken an oral JAK inhibitor before enrolling. The four who had responded to their most recent JAK inhibitor (at least a 30% SALT improvement) showed a 48.3% mean SALT reduction on bempikibart; the six who had not responded showed 4.8%. JAK-naive patients showed 44.1%. With four and six patients these are small exploratory subgroup estimates, not reliable comparative evidence; patients who fail a JAK inhibitor are an obvious audience for a new mechanism, and in this sample bempikibart did little for them.[15]
The slides report largest median reductions from baseline through Week 36 of 44% in CD3+ T cells, 52% in eosinophils and 39% in TARC, with 41% to 46% falls in interferon-gamma, IL-17 and IL-6 among patients with high baseline levels. CXCL10, a marker downstream of interferon-gamma, fell by a median of 32% in eight responders versus 3% in twelve non-responders (p=0.016, Mann-Whitney), an exploratory comparison that the slides note was not adjusted for multiplicity. One illustrated biopsy showed fewer T cells around the follicles and more growing follicles at Week 36. The data fit the intended IL-7 and TSLP mechanism; they do not predict who will respond.[16]
Q32 Bio EADV late-breaking presentation, September 30, 2026 · EADV poster AS-726
Bempikibart, formerly known as ADX-914, is a fully human monoclonal antibody directed against the alpha chain of the interleukin-7 receptor, IL-7Rα. The receptor is shared by the IL-7 and thymic stromal lymphopoietin pathways. Blocking IL-7Rα is therefore designed to interrupt two upstream immune signals implicated in T-cell survival, activation and inflammatory disease biology.
Alopecia areata is driven by an immune attack on the hair follicle. Current oral JAK inhibitors intervene downstream in intracellular cytokine signaling. Bempikibart instead aims to modulate an upstream receptor with a subcutaneous biologic. The aim is efficacy plus a cleaner safety profile, limited monitoring and infrequent maintenance dosing.
Q32 is positioning bempikibart as a selective, non-JAK biologic that could deliver meaningful regrowth without the class-related boxed-warning framework carried by approved oral JAK inhibitors. That is a development objective, not an established label advantage: only a large controlled safety database and eventual regulatory review can determine the final risk language.
Q32 regained full development and commercial rights to bempikibart when its collaboration with Horizon was terminated in November 2023, after Amgen acquired Horizon. The termination agreement originally carried regulatory and sales milestone payments to Horizon of up to $75.1 million. On November 7, 2025, an amendment with Amgen replaced them with a one-time grant of 553,695 Q32 shares, and the second-quarter 2026 Form 10-Q states that Q32 has no remaining obligations to Amgen, including those milestone payments. Bempikibart’s future economics are therefore not encumbered by that legacy agreement.[18]
Q32 announced FDA Fast Track designation for bempikibart in alopecia areata on April 30, 2025. This facilitates regulatory interaction and does not establish approval. Q32 Bio investor document (PDF).
September 30 update: the first half of 2027 was restated for the registration-directed program in severe or very severe AA, subject to regulatory discussions planned for the second half of 2026, and the EADV slides call it a Phase 3 pivotal trial. No agreed pivotal protocol, regulatory authorization or trial initiation has been announced.

For scale, Nektar’s ZENITH AA will enroll 850 patients with SALT≤20 at Week 52 as primary endpoint; Q32 Bio has not disclosed its own design.[19]
ClinicalTrials.gov NCT06018428 remains active, not recruiting in the record last posted May 8, 2026 (as of October 2, 2026), with 75 estimated participants across the study and estimated completion September 2026. No results are posted in the registry.
ClinicalTrials.govCompetition also includes investigator-sponsored Phase 2 trial NCT05551793, evaluating dupilumab in alopecia areata with atopic background and/or elevated IgE. Its June 17, 2026 registry update listed recruitment and estimated enrollment of 76; the study is not a positive clinical result. ClinicalTrials.gov record NCT05551793.
Alopecia areata can range from patchy hair loss to complete scalp loss and loss across the body. Q32 cites approximately 700,000 people living with the disease in the United States; its July presentation used a broader estimate of approximately 800,000, with roughly 530,000 potentially addressable and approximately 300,000 treated with advanced therapies. These are company estimates, not a revenue forecast.
The competitive benchmark has become more demanding. Three oral JAK inhibitors are available in the United States for severe alopecia areata: baricitinib, ritlecitinib and deuruxolitinib. Their existence validates the commercial market and the SALT endpoint, but it also means bempikibart must show a clear reason for physicians and patients to choose an injectable biologic.
| Therapy | Company | US status | Route / population | Relevance to bempikibart |
|---|---|---|---|---|
| Olumiant (baricitinib) | Eli Lilly / Incyte | FDA-approved in 2022 | Oral; adults with severe AA | First systemic approval and an established efficacy benchmark. |
| Litfulo (ritlecitinib) | Pfizer | FDA-approved in 2023 | Oral; adults and adolescents age 12+ | Sets a pediatric/adolescent access benchmark. |
| Leqselvi (deuruxolitinib) | Sun Pharma | FDA-approved in 2024; US launch announced in 2025 | Oral; adults with severe AA | Adds another effective oral option and intensifies commercial competition. |
| Rezpegaldesleukin | Nektar Therapeutics | Phase 2b completed; ZENITH AA Phase 3 to start in early 2027 | Injectable regulatory T-cell stimulator; severe to very severe AA | Closest peer in timing and positioning. |
| Bempikibart | Q32 Bio | Phase 2a; registration-directed program planned | Subcutaneous biologic; severe/very severe AA studied | Potential differentiation through target selectivity, safety, durability and maintenance convenience. |
Nektar Therapeutics’ October 1, 2026 release is the most relevant competitive event since the July readout. REZOLVE-AA was a placebo-controlled Phase 2b study in 92 patients with severe to very severe disease. In a blinded extension from Week 36 to Week 52, 29% of patients on the low dose and 31% on the high dose achieved new SALT≤20 responses, compared with none on placebo. Among patients treated for 52 weeks, 6 of 8 (75%) kept SALT≤20 four months after stopping, and 5 of 8 (63%) six months after stopping. ZENITH AA, an 850-patient Phase 3 with SALT≤20 at Week 52 as the primary endpoint, is set to start in early 2027.[19]
The programs are not directly comparable. Nektar’s initial study was randomized but missed its original AA primary endpoint; the off-drug follow-up of eight selected patients has no contemporaneous placebo control. Q32’s dataset is single-arm. Neither durability subset establishes comparative efficacy.
Cross-trial comparisons are especially hazardous here. SIGNAL-AA Part B was small and open-label, while approval trials for the JAK inhibitors were randomized and placebo-controlled, used different treatment durations and enrolled different populations. The correct late-stage question is not whether one topline percentage looks numerically larger than another. It is whether bempikibart can reproduce clinically meaningful SALT responses versus placebo with an attractive risk-benefit profile and practical dosing.
The September 2019 BMS license remains relevant: the June 2026 filing lists contingent development/regulatory and sales milestones, tiered royalties reaching 10% of net sales, and sublicense-income obligations. Q32 paid an $8 million upfront fee under the license and, in July 2024, a $4 million development milestone; it reported no subsequent payment-triggering event through June 30, 2026. Q32 Bio quarterly report for the period ended June 30, 2026 (SEC).
The Q2 filing reports $106.3M cash and equivalents at June 30, before the July offering. The July 16 closing added $187.6M net after fees and expenses. The simple sum, about $293.9M, is an illustrative bridge, not a reported current balance; the June figure already includes the May financing and the debt repayment.

H1 operating cash use was $9.418M versus $23.133M in H1 2025. Q2 EPS was a loss of $0.44. Management now guides resources through topline Phase 3 results of the planned registration-directed program, a milestone-based forecast without an assigned readout date.
Second-quarter research and development fell to $4.3 million from $5.2 million a year earlier, as ADX-097 costs disappeared after the sale to Akebia, partly offset by $0.6 million more bempikibart spending on manufacturing and nonclinical work. Accumulated deficit reached $221.5 million at June 30, 2026. The 10-Q also states that existing cash will not fund any program through regulatory approval: the guidance covers a Phase 3 readout, not a launch.

The Q2 subsequent-events note confirms $187.6M net proceeds; the $200M headline is gross. The 30-day option for 1,643,835 extra shares has lapsed with no confirmed exercise, so no option cash or shares are counted.
The July prospectus’s $75M remaining ATM capacity and 45-day restricted period are historical terms; the restriction no longer applies as of October 2, 2026, and ATM capacity is not committed cash.
SEC · July financingThe August 3 cover count is 29,769,466 common shares, more current than the July prospectus’s 29,708,814 projection. At the October 1, 2026 close of $8.25, basic equity value is approximately $245.6M. Some market-data services still show a stale 23.74M share count, the June 30 figure, which should not replace the filed count.
Basic equity value: $8.25 Marketstack October 1, 2026 close × 29,769,466 SEC common shares (August 3). Pre-funded equivalents are analyzed separately.
The June filing shows 1,175,654 older pre-funded warrants unexercised at June 30; July added 4,931,506. Adding these dated warrants to the August common count yields an illustrative 35,876,626 share-equivalent base (~$296.0M at $8.25), assuming no intervening warrant exercises already embedded in that count. It is not a verified fully diluted count.
The July 28 resale registration covers 6.875M May PIPE securities, including 150,000 pre-funded equivalents. Q32 does not receive proceeds from secondary resales. Registration is not evidence that holders sold, nor a new second issue of the same May shares.
SEC · share count and warrantsQ32 sold substantially all assets related to ADX-097 to Akebia Therapeutics in November 2025. The consideration included $7 million at closing, $3 million at the six-month anniversary and $2 million upon the earlier of the first milestone or December 31, 2026. Beyond those $12 million of upfront and near-term payments, Q32 is eligible for up to $580 million of additional development, regulatory and commercial milestones and tiered royalties ranging from low single digits to mid-teens on potential annual net sales.[10]
The 10-Q confirms the $3.0 million payment on May 28, 2026, which leaves the $2.0 million due by December 31, 2026 as the only dated cash inflow in the calendar. Of the $580 million of contingent milestones, up to $92.5 million relates to development and regulatory events and up to $487.5 million to commercial ones, so most of it depends on Akebia bringing ADX-097 to market.
Q32 retained its tissue-targeted complement platform, including ADX-096 and other early-stage assets, and has said it is evaluating strategic options. Bempikibart nonetheless remains the dominant value driver.
Q32 Bio’s investor-relations site lists seven firms covering the stock as of October 2, 2026: Cantor Fitzgerald, Clear Street, H.C. Wainwright, Jefferies, Morgan Stanley, Oppenheimer and Wells Fargo. Individual firm targets are not listed because the underlying notes are not public documents that readers can check.[20]
In this hub’s interpretation, the July data support further development and the financing reduces near-term funding pressure. The coverage list alone does not establish a consensus view or clinical probability.
July filings also include institutional disclosures from Opaleye, Carlyle/Abingworth, Boxer and OrbiMed, Manke’s exercise of 53,521 options, PSU awards for Morrison, Kalowski and Violette, and eight director Form 4/A corrections. The PSU financing condition was met, but quarterly service vesting over three years continued. The amendments corrected June awards to 10,826 options per director rather than recording eight fresh July grants. None of these sixteen filings reports an open-market sale. StockTwits · $QTTBAccording to Q32 Bio, Trevejo brings more than 20 years of experience, including CMO roles at Enveda and Tarsus; Hershberger brings more than 30 years, including leadership at Adagio/Invivyd and Visterra. These biographies are company-reported. The September 14 Form 8-K adds that Trevejo was Chief Medical Officer at Tarsus from February 2022 to February 2024, when XDEMVY was approved and launched, and that Hershberger, as Chief Development Officer of Adagio (now Invivyd) from June 2020 to January 2023, took a monoclonal antibody from IND-enabling work into Phase 3. Trevejo is a co-author of the EADV late-breaker and is quoted in the September 30 release on moving “rapidly into registrational studies in the first half of 2027.”

Q32 Bio is led by chief executive officer Jodie Morrison. Lee Kalowski serves as president and chief financial officer, and co-founder Shelia Violette, Ph.D., serves as chief scientific officer. Recent execution includes the Part B readout, the ADX-097 sale, several financings and the repayment of venture debt.
The May $55 million private placement was led by BVF Partners with participation from RA Capital Management, OrbiMed and Atlas Venture. The July 28 resale registration means those securities can become market supply.
With roughly $188 million of confirmed net proceeds, the question shifts from whether Q32 can finance the next step to whether management can turn that capital into a well-designed, regulator-aligned program.
The development plan now has three evidence stages. The initial Week 52 off-drug findings were presented at EADV on September 30, 2026. Part B open-label extension data are guided for the second half of 2027. A registration-directed program is planned to start in the first half of 2027 after regulatory discussions in the second half of 2026. Enrollment and dosing in the OLE are ongoing.
The September 30 release states that the off-drug and OLE analyses will inform a long-term chronic maintenance regimen that could include monthly, bi-monthly or quarterly dosing, to be evaluated in future studies, and the EADV slides conclude that the maintenance dose is “predicted to be monthly-quarterly.” It remains a prediction from 12 responders followed for 16 weeks.
ADX-914-XL is a half-life-extended anti-IL-7Rα candidate in preclinical development. The intention is longer dosing intervals, not demonstrated clinical equivalence to bempikibart.

Part B had no placebo arm. Spontaneous regrowth and selection effects cannot be separated from drug effect.
Only 33 patients were enrolled and 25 were included in mITT efficacy analyses. The SALT-20 rate is 40.0% in mITT but 30.3% in ITT. Late-stage success depends on reproducibility across a much larger population.
Q32 retains other assets and ADX-097 economics, but bempikibart remains the dominant controlled clinical program. A pivotal failure, safety issue or regulatory delay would affect most of the current thesis.
Basic shares more than doubled versus March 31 after the May PIPE, ATM and July common issuance. New and older pre-funded warrants, 2,252,380 options outstanding at June 30, RSUs and the remaining ATM create additional share-equivalent and future-supply considerations; the underwriter option from the July offering has lapsed without a confirmed exercise.
Approved oral JAK inhibitors have established prescribing and payer pathways; an injectable must offer clear differentiation.
Premature expansion into new populations or indications could raise burn and distract from the core program.
ADX-097 milestones are not guaranteed assets: apart from the $2.0 million due by December 31, 2026, they depend on Akebia’s development and commercial success. The legacy Horizon milestone obligations on bempikibart were settled in shares in November 2025 and no longer apply.
In the mITT population, six patients who had not responded to their most recent oral JAK inhibitor showed a 4.8% mean SALT reduction. If the pattern holds, it narrows bempikibart’s positioning.
Nektar plans to start an 850-patient Phase 3 of rezpegaldesleukin in early 2027. Two similar programs could compete for sites and patients.
Q32 Bio is in a materially stronger position than it was before July 13. Bempikibart produced a credible Phase 2a activity signal in severe and very severe alopecia areata, with a favorable reported safety profile and early examples of off-drug durability. The July offering raised approximately $200 million gross at $18.25 per share-equivalent. Venture debt had already been repaid during the first half of 2026, before that offering.
The updated QTTB equation: encouraging open-label efficacy + favorable early safety + a fortified balance sheet, minus unresolved placebo-adjusted efficacy + registrational-design risk + substantial denominator expansion. The next decisive evidence will come from FDA alignment, full durability data and a controlled late-stage program—not from the Phase 2 headline alone.SEC · Q2 2026 · IR · September 1

No. Part B was an open-label, single-arm Phase 2a study, so its results cannot be placebo-adjusted.
Q32 Bio plans a registration-directed program, called a Phase 3 pivotal trial in its EADV slides, in the first half of 2027, after regulatory discussions planned for the second half of 2026. No exact date has been announced.
It reported $106.3 million at June 30, 2026, before $187.6 million of net proceeds from the July 16 offering, and guides that its resources fund operations through topline Phase 3 results.
No. The up to $75.1 million of legacy milestones were replaced in November 2025 by a grant of 553,695 shares; the Q2 2026 10-Q reports no remaining obligations to Amgen.
