LiGeR-HN1; October 28, 2026
Where Genmab stands now
The October 28 LiGeR-HN1 result adds randomized survival evidence for petosemtamab plus pembrolizumab. It follows positive RAINFOL-02 and EPCORE DLBCL-2 readouts in different disease settings. Existing DARZALEX royalties remain central to earnings while Genmab builds a larger owned portfolio. Detailed efficacy and safety, regulatory decisions, cash conversion and borrowing costs are the next determinants.
Positive topline results still require regulatory and commercial execution. Investigational Rina-S and petosemtamab are not approved products. Acquisition debt, development spending and partnership profit-sharing affect how clinical success may reach common shareholders.
The constructive case
Positive randomized data in petosemtamab and Rina-S broaden the potential owned portfolio, while the existing royalty business provides revenue and earnings. The next test is consistent full data, regulatory progress and productive launch spending.
The cautious case
Acquisition debt, weaker cash conversion and rising launch costs remain relevant after positive trials. Investigational medicines still require approval and commercialization, and broader sales opportunities do not guarantee attractive returns on invested capital.

Genmab late-stage evidence: separate trials and populations
On October 28, 2026, Genmab announced that the Phase 3 LiGeR-HN1 trial met its dual primary endpoints of overall survival and objective response rate at the first interim analysis. The study compares petosemtamab plus pembrolizumab with pembrolizumab alone in first-line recurrent or metastatic PD-L1-positive head and neck squamous cell carcinoma. It is randomized, open-label, global and multicenter, with patients assigned one-to-one to the combination or control arm.
Rinatabart sesutecan, also called Rina-S or GEN1184, is an investigational folate receptor alpha-targeted antibody-drug conjugate. Its components include a human monoclonal antibody, a protease-cleavable linker and exatecan, a topoisomerase I inhibitor payload. The drug is being developed across ovarian, endometrial and other cancers, but the October 23, 2026 pivotal result concerns platinum-resistant ovarian cancer in RAINFOL-02.
GMAB royalties, acquisition debt and cash conversion
Cash and cash equivalents were $1.503 billion at June 30, 2026, compared with $1.715 billion at December 31, 2025. The June balance included $30 million of restricted cash held in escrow in connection with ProfoundBio. Cash reported on the balance sheet and cash freely deployable for operating decisions are therefore not perfectly identical. The company generated $54 million of operating cash flow in the first half of 2026, down from $349 million a year earlier. The cash-flow statement shows $317 million before financial items, followed by interest received, lease interest, $191 million of interest paid and $96 million of corporate taxes paid. Receivable growth and a reduction in other payables also absorbed cash. Positive operating profit did not translate into an equally large increase in cash.

Red flags
- Investigational Rina-S and petosemtamab are not approved products. Acquisition debt, development spending and partnership profit-sharing affect how clinical success may reach common shareholders.
- Acquisition debt, weaker cash conversion and rising launch costs remain relevant after positive trials. Investigational medicines still require approval and commercialization, and broader sales opportunities do not guarantee attractive returns on invested capital.
GMAB Stock FAQ: Genmab Questions Answered
What changed in the October 28 petosemtamab announcement?
LiGeR-HN1 met both primary endpoints at its first interim analysis: overall survival and objective response rate. The overall-survival hazard ratio was 0.50, with a 95% confidence interval of 0.37–0.67, for petosemtamab plus pembrolizumab versus pembrolizumab in first-line PD-L1-positive recurrent/metastatic HNSCC. Petosemtamab remained investigational.
Is Rina-S already approved after RAINFOL-02?
No. The October 23 announcement described a positive Phase 3 result and plans for regulatory discussions. Rina-S remained investigational. An approval requires a separate regulatory decision on the complete application and proposed indication.
Are the RAINFOL-02 ITT and PAS results from different trials?
No. The ITT population contained 584 randomized patients; the 492-patient PAS was a defined subset incorporating folate receptor alpha expression and prior mirvetuximab criteria. PFS hazard ratios were 0.29 in the ITT population and 0.33 in the PAS; overall-survival hazard ratios were 0.43 and 0.44 respectively.
Does Genmab receive all EPKINLY/TEPKINLY global sales?
No. First-half 2026 global sales were $312 million, but Genmab recorded $242 million of EPKINLY product sales in the United States and Japan. Sales in other territories contribute under royalty arrangements, while the company also has profit-sharing obligations to AbbVie.
How much cash and borrowing did Genmab report?
At June 30, 2026, cash and cash equivalents were $1.503 billion, including $30 million held in escrow. Borrowings totaled $5.135 billion, combining current and noncurrent amounts. These figures describe that balance-sheet date, not a live cash balance.
